A study of a small sample of construction fatal accidents was used to pilot a potential method of analysis for the UK Health and Safety Executive (HSE) Construction Division. The work was conducted in the context of a Governmental Inquiry into construction fatalities in 2009, but has been developed further since. The sample of 26 accidents (28 fatalities) was drawn from the 211 fatal accidents in the years ending 2006–2008, to be broadly illustrative of the range of accident characteristics. The accidents were analysed on the basis of available inspectorate reports and structured interviews with the investigating inspectors. A standard method of classification on four levels was developed, based on the Human Factors Analysis and Classification System (HFACS) classification of errors and task level factors, with additional categories covering the organisational and regulatory/market levels of the system. The results showed a concentration of underlying factors associated with inadequacies in planning and risk assessment, competence assurance, hardware design, purchase and installation, and contracting strategy. These findings were partially validated by comparing them with another sample of 50 accidents analysed earlier by the HSE. This paper describes the development and testing of the investigation and classification method and how it is being further developed since the initial study. Helpers can gain future indirect fitness benefits by increasing the survival of breeders that produce offspring related to the helper. Helping augments group size through the helper's presence and, in some cases, by increasing fledging success. Breeders may then experience enhanced survivorship because of the benefits of living in large groups. Helping may also reduce the workload of the breeder, which in turn may increase the likelihood that the breeder will survive to breed again. We used Cox's proportional hazards model to examine whether breeders' survival in two populations of the red-cockaded woodpecker (Picoides borealis) was enhanced when group size was increased in the presence of (1) the helper itself, or (2) "extra" fledglings (fledglings produced by the breeder because of helping behavior). We found that in the presence of helpers, the risk of a breeder dying declined by 21–42% for males and 0–14% for females. Our results suggest reduced breeder workload as one mechanism to explain reduced breeder mortality in the presence of helpers: breeders spent less time incubating and provisioning nestlings when assisted by helpers. The risk of a breeder dying declined by 16–42% in males and 26–43% in females in the presence of "extra" fledglings. We speculate on possible mechanisms by which fledglings might affect breeder survival. Our results support the hypothesis that helpers gain future indirect benefits by reducing breeder mortality. Head neck and back injury represents 32% of injuries in the NSW coal mining industry. There is considerable hearsay evidence that rough rides in vehicles are a significant cause of such injuries but no objective studies have been carried out.Analysis of workers compensation data showed that overexertion and slips and falls were the most significant causes of head neck and back injuries but approximately 11% of all head neck and back injuries were reported to be due to vehicle jarring. Two types of underground vehicles accounted for 53% of all injuries. These were Shuttlecars and Underground Transporters.There was no evidence from the compensation data that whole body vibration was a significant factor in head neck and back injuries.  相似文献
